It’s important that you keep us informed of any changes to your contact details, address or personal details such as:
- Change of phone number (landline or mobile)
- Change of address
- Change of email address
- Change of name
If you don't inform of any changes to your details at the earliest opportunity there is a risk that you will miss important information about your care such as hospital appointments or test results, or that information about you may be received by someone else.
The quickest and easiest way to update your details is to email us at firstname.lastname@example.org making sure you include your:
- Your full name
- Date of birth
- Old details
- New details
If you believe we have your Date of Birth recorded incorrectly please provide evidence of your correct date of birth and we can change this for you
